This estimate will outline the anticipated costs for medical procedures, tests, medications, equipment, and facility fees.

You should receive this estimate in writing at least one business day before your scheduled service. You can also request an estimate before making an appointment.

Protecting Yourself from Unexpected Costs:

If the final bill is $400 or more above the provided estimate, you have the right to dispute the charges.

Keep a copy of your Good Faith Estimate for your records.
